Initial Assessment #1–2
$232.50 (per assessment)
The first initial assessment will include
the following:
-Initial welcome phone call from the assigned OT to gather background information to prep for the initial assessment #1 and #2 visit. (approx 15-20 min).
-60 minute in person session to meet your child/youth plus 10 minutes indirect time for documentation/prep/clean up.
The second initial assessment will include the following:
Gathering and scoring of any relevant sensory assessment questionnaires or motor skill assessments to summarize these either verbally or in an email to the family.
-60 minute in person session scheduled with the child/youth. In addition, the therapist will need indirect time to review any other reports shared, and to prep/clean up the space, and do documentation.
-Verbal or email follow up at the end of the initial assessment #1 and #2 to indicate plan for therapy going forward based on the assessment findings.
50 Minute Therapy Intervention Session + 10 Minutes for clean up/documentation (In-Person or Virtual)
$155.00 (per intervention)
In-Person Appointment
These appointments will include the following:
-50 minute therapy intervention sessions booked in person with the child or youth, with 10 minutes being billed for clean up after session and documentation.
Virtual Appointment
Virtual therapy intervention sessions will include:
-Google Meet link sent to the client and family for the virtual therapy session. (50 minutes)
-10 minutes of indirect time billed for therapist prep and documentation.
50 minute OT Parent Consultation + 10 minutes of indirect time for prep/documentation (In-Person or Virtual)
$155.00 (per consultation)
Therapy parent consultations will include the following:
T-50 minute phone call or in person with the occupational therapist + 10 minutes of indirect time for therapist prep and documentation.
Thrive Parent Hub Webinars and Workshops

Parent Education Session/Webinar will include the following:
